REFERRAL AND ASSESSMENT
Referring Agencies or individuals are able to obtain referral forms from Muloobinba Aboriginal Corporation at 12 Bishopsgate St, Wickham, or contact the Substance Misuse Caseworkers on 0249695299.
Completed Referral forms are to be received by Muloobinba by close of business Mondays via fax, email or hand delivered.
Initial Assessments will be conducted on Tuesdays, which can be done in person or over the phone. Substance Misuse Caseworkers will contact the applicant and/or the referring agency to organise a time for the assessment to be finalised. Applicants and the referring agency will be informed of the outcome of the assessment as soon as all relevant information is discussed.
All applicants and referring agencies will be provided with relevant information concerning the outcome of the assessment.
Once accepted into the program, the Substance Misuse Caseworkers will provide participants with information regarding the dates, times, and venues of the program.
